We offer a membership model to allow you the greatest access and value to our comprehensive services. If you are not interested in membership, we still offer fee-for-service. See pricing for our Restorative Services Menu and our Membership Models.

The CIA is $250 and consists of 3 appointments

At FGHC, we have created a collaborative intake process which is a comprehensive model that allows us the time needed to get to the root of your symptoms through various assessments and analysis. This process takes a series of four appointments. Learn more about our process here.
We have a strict cancellation policy. We ask a minimum of a 24 hour/business day/s notice to reschedule an appointment without a $35 fee. All appointments must be rescheduled by calling the front desk during normal business hours at 352-512-0631 or the cancellation will immediately be pushed to the following day and be subject to our policy.
Due to the unique model of care that we provide, we thank you for your understanding and support as we strive to provide excellent service and care to each of our clients. We are only able to schedule one client per hour and there is often preparation for each scheduled appointment. So when an appointment is missed and it is not an emergency, our providers miss opportunities to serve clients that are often waiting to be seen sooner.

All of your intake paperwork given to you in your client portal must be completed 24 hours prior to your first appointment. This includes your brain health assessment, temperament assessment, HRV, and InBody Scan. Please be sure you are scheduled for these.
This allows our providers time to review your records before they meet with you.
If you choose to become a Direct Primary Care member, you can not submit office visit claims to insurance, however, lab tests and diagnostic imaging is still covered under your policy.
For counseling, our model is faith-based and functional, focused on root causes vs. symptoms so we do not currently offer diagnostic codes that are required for you to be reimbursed by insurance.

We use traditional laboratory services, like those offered from Quest Diagnostics, as well as functional diagnostic testing. We work with you to prioritize diagnostic testing based on your symptoms and health goals.
Yes, while we encourage a 12-month contract, you can cancel your membership at any time with a 30-day notice signed cancellation form requested from our front desk.

We allow service dogs only in our building. Also, we have two comfort dogs, Ben and Nole, that are often on duty at our health center.
